I have voluntarily decided to participate in activities offered by Brazilian Fight Factory, LLC. I do hereby waive, release, and forever discharge Brazilian Fight Factory, LLC from liability pertaining to matters set forth below. I understand by signing this Waiver and Release, I expressly and willingly agree to assume complete responsibility for any risk of injury that may arise from the said activities. On behalf of myself, my heirs, assigns, and next of kin, I waive, release, and forever discharge Brazilian Fight Factory, LLC, and/or its respective owners, directors, employees, contractors, landlords, members, and/or any party under or affiliated with Brazilian Fight Factory, LLc of any and all claims for loss, damages, injuries, and death sustained to me or my property that I may against Brazilian Fight Factory, LLC. I understand that the activities that I will participate in are inherently dangerous and may cause serious injuries, including bodily injury, damage to personal property, and or death. By this waiver, I assume any risk and take full responsibility any waive any and all claims of personal injury, including severe bodily injury, damage/loss to personal property and death relating to all activities associated with Brazilian Fight Factory, LLC, including but not limited to receiving lessons at the facility, using the facility and its equipment, practicing and engaging in martial arts activities and related activities on and off school premises. If I am injured from said activity, I release, waive, forever discharge, covenant not to sue, and will not hold liable Brazilian Fight Factory, LLC and/or its respective owners, directors, employees, contractors, landlords, members, and/ir any party under or affiliated with Brazilian Fight Factory, LLC.
I will obey all Rules and Regulations of Brazilian Fight Factory, LLC and understand that Brazilian Fight Factory, LLC is not responsible for any injury (or loss of property) to any person suffered while participation in activities offered by Brazilian Fight Factory, LLC for any reason whatsoever. I grant Brazilian Fight factory, LLC its representatives and employees the right to take photographs of me and my property in connection with said activities. I authorize Brazilian Fight Factory, LLC, its assigns and transferees to copyright, use and publish the same in print and.or electronically. I agree that Brazilian Fight Factory, LLC may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and web content. I have read and fully agree to the terms of this waiver and release. I understand and confirm that by signing this waiver and release I have given up considerable future legal rights. I have signed this waiver freely, voluntarily, under no duress, or threat of duress, without inducement, promise, or guarantee being communicated to me. My signature is proof of my intention to execute a complete and unconditional waiver and release of all liability to the full extent of the law.
